Covid Release Form

It is mandatory that all participants and staff complete the below waiver.  The document must be reviewed and signed by parents/legal guardians of all minors or those over the age of 18 may sign for themselves.  There will be no exceptions.  If a completed form is not on file, the participant will not be permitted to participate.  Please bring the completed form with you on day 1 of the tournament for verification.
